Deverbal from injertar or semi-learned borrowing from Latin īnsertus (literally “inserted”), later "grafted" by confusion between homonymous īnserō (“to insert”) (perfect passive participle īnsertus) and īnserō (“to graft”) (perfect passive participle īnsitus). Compare also enjerto and inserto, from the same source.
