Fruitcake as Insanity

I see that we draw a connection between fruitcake as insanity and the phrase "nutty as a fruitcake". I would like to get a bead on the types of "very nutty" fruitcakes we are taking about in this colloquialism. Most fruitcakes I have known did not necessarily have that many nuts in them, although I don't claim to be an expert.
