Usually explained to be a reference to the typical distance between the computer monitor and the user's face, in centimetres.

      Thus, it is a sort of ”Error 40", where the error is not in the machine or in the numbers themselves, but approximately 40 cm in front of the screen – that is, with the person using the machine and interpreting its output.
