anh (elder brother) +‎ hai (two); literally "the second eldest brother".

Folk etymology claims that in the times when Northern and Southern Vietnam were separated, families in the South often had the eldest son to reside in Northern Vietnam, while the second eldest son lived with the rest of the family in the South and therefore became the "eldest son".
